       Info needed from callers: Basic info requested from callers: Name of caller, call  back number, regarding, On death calls, we typically get the following info; Name of caller, phone number, facility, name of deceased, age, location of body, if a residential call- the address, next of kin, relationship, phone number, if family is  present. If you need additional info, please make note in the space provided.

Obits are requested at LEAST 30 minutes before you forward your lines to AnswerTel.
Due to the highly increased chance of error and the amount of operator time needed, please DO NOT give verbal obituaries. Verbal obits are subject to a $1.00 per obit charge. Information we need is: name of deceased, day/time/place of burial (no flowers, donations to local hospice, etc.)

Each director has his/her preferences for being contacted. Please fax a list of your staff and please include their name, numbers and the order they wish to be contacted. This is very important for the answering service to know how to reach the proper person.
